Form SS-4 — Application for Employer Identification Number (EID#)

Prepare and process to the appropriate Internal Revenue Service District office for your specific area after you have incorporated.

Form 2848 — Power of Attorney

Prepare a Power of Attorney to allow Small Business Solutions, Inc. to obtain copies of all communications between the IRS and the client. The Power of Attorney is limited so that Small Business Solutions, Inc. is not authorized to act on behalf of the taxpayer without prior written authorization.

Form 2553 — Election to be an "S" Corporation

Complete Form 2553 as instructed by the owner(s) of the company. Small Business Solutions, Inc. will assist the client in evaluating the various options available for their personal circumstances. The election has to be made within seventy-five (75) days of when the company first had assets or first started doing business, whichever comes first.

Services Related to Using as an "S" Corporation

Telephone support for up to 30 days for questions relating to "S" corporation type transactions.

Organize Files

Compile the new company's tax and corporate documents (provided by the company's attorney) into a Permanent Reference file and to provide you with a complete set of the same documents.

New Employee File in QuickBooks Pro ®

Assistance with the setup of QuickBooks Pro 2003®, the employee master records, the related payroll tax accounts, and then linking the master file to the specific account classifications in the chart of accounts.

Year of Converting

Chart of Accounts/Data File — Set Up/Prepare/Edit

Create a QuickBooks Pro 2002® and 2003® data file utilizing one of the various charts of accounts that have been developed by Small Business Solutions, Inc. Once the new client has completed the "New Client Information Data Sheet," your chart of accounts will be modified for your specific needs.
